He says: "Pray Gentlemen be merciful- consider I have some feeling." Melville stands holding his patient's left hand, his knife in the right; he turns his head in profile to the right. to look down at the kneeling Trotter, saying, "Thats right Mon! Ye as my confidential Agent Catch aw the bluid. then they will na suspect me of the Operation." He wears a plaid and Scots cap in which is a thistle held by a brooch on which is a St. Andrew's cross.
